What is a gynecomastia?

Gynecomastia refers to enlargement of the male breasts, resulting in a feminine chest appearance in men which could seriously impact an individual’s self confidence.

It is a condition that occurs as a result of increased fat collection in the breasmbination of the three.

Men can experience gynecomastia for many reasons usually beginning during adolescence as a natural part of puberty, or it may be due to some other causes like significant weight loss, weight fluctuations, certain medications intake, hormonal imbalances, genetics, Marijuana, or anabolic steroids use. 

For most patients there is no clear cause for this condition.

Gynecomastia is a general term that is used to describe any breast enlargement no matter what combination of tissue excess causes the enlargement. 

Pseudogynecomastia on the other hand refers to gynecomastia conditions caused by accumulation of excess fat collection only, which is commonly seen in men with obesity.

 

Gynecomastia has many different classifications and staging systems however the most commonly used classification divides gynecomastia to 4 different grades:

  1. Grade I: small breast enlargement without excess skin (localized to the nipple and areola)
  2. Grade II: moderate breast enlargement without skin excess (outside the areola)
  3. Grade III: moderate breast enlargement with some skin excess (the whole breast)
  4. Grade IV: marked breast enlargement with sever skin excess and breast ptosis (mimicking a female breast with the nipples fallen down and pointing to the ground)

 

What is male breast reduction surgery?

Male breast reduction surgery is a general term that refers to managing gynecomastia either surgically or by liposuction according to the gynecomastia grade.

  1. Grade I: requires a small periareolar incision and glandular tissue excision.

 

  1. Grade II: requires a Vaser ultrasound device liposuction with the possibility of glandular tissue excision as in grade I.

 

  1. Grade III: requires surgical circumareolar incision + liposuction or surgical fat removal + circumareolar skin tightening in most cases (hidden scar).

Grade IV: requires surgical breast removal (mastectomy) resulting in 2 permanent transverse scars that lie across the chest, one on the right and one on the left.

Recovery from breast reduction surgery:

All breast reduction surgeries are simple procedures that have short recovery duration.

Drains (small plastic tubes) could be required in some cases to drain any remaining fluids after surgery, and in such case they are removed few days after surgery.

Sutures are usually of the absorbable type that require no removal however if the non absorbable sutures are used they are removed by the 14th day after surgery.

Going back to work is expected by the 10th – 14th day after surgery, 

Going back to gym after about 1 month, 

Full recovery is expected by 3 months.
